1. Your doctor's written referral, if you have one.

2. Your insurance referral, if required.

3. All x-rays, CT scans, and MRIs. Please bring the actual scans. 

4. A list of all of your medications.

5. All pertinent blood test results.

6. Your insurance cards and identification.

7. Your doctor's name, address and phone number.

8. Your completed patient registration forms, if you printed it at home.

Our practice participates in Medicare, Blue Cross and most major HMO’s and PPO’s.  We will submit claims for the services rendered to the insurance company for you.  You will be responsible for services not covered by insurance or any balance after the insurance company has made payment.  Copayments are due at the time of service.  If you receive a bill and you have questions, please contact our new billing office at 866-243-7105.  Our billing manager will be happy to answer any questions you may have.
