Once you've made your first appointment at one of our Montgomery County, Maryland, ENT offices, you should gather the following items and bring them along with you. These will help make your registration run more smoothly:
- An insurance referral and a written referral from your doctor, if required by your insurance
- Any X-rays, MRIs, CT scans
- A list of the medications you are currently taking
- Blood test results that are relevant to your condition/symptoms
- Insurance cards and ID
- The address and phone number of your primary or referring physician
- Patient registration form (s), if you completed them at home

Three registration forms are conveniently provided on the Patient Registration page. If you can, please take the time to complete these forms prior to your Montgomery County, Maryland ENT appointment. Adult patient information, patient health history, and disclosure forms are easy to select, print, and complete. There is a separate registration form for dependent children. If it has been more than twelve months since you have visited this Montgomery County Ear, Nose, and Throat specialist group, please fill out all three forms and bring them with you. Once your personal and medical information has been updated in our system, it will be that much easier to provide seamless handling of your insurance billing and to better provide you with quality health care service.
