Our practice participates in Medicare, Blue Cross and most major HMO’s and PPO’s.  If your insurance requires a referral, it is due at the time of your visit. We will submit claims for the services rendered to the insurance company for you.  You will be responsible for services not covered by insurance or any balance after the insurance company has made payment.  Copayments are due at the time of service.  If you receive a bill and you have questions, please contact our billing office at 866-243-7105.  Our billing manager will be happy to answer any questions you may have.
